Transcript Request Form
Transcripts can be copied and sent to colleges,  scholarships, and other agencies that require them. Transcripts to the State of Florida University System (FUS) are sent electronically. If you are required to enclose the transcript in your application package, you can pick it up within a week after your request. Otherwise, all transcripts are mailed so that they are considered "official."
 
     
Important!

If you are applying to a private college, chances are very good that a Secondary School Report is required from your counselor, along with a letter of recommendation.
Please see you counselor about this after you request the transcript.
